Community Health Partners is a Florida Physician Hospital Organization (PHO) that represents a large network of physicians, hospitals and other medical providers in Collier and southern Lee counties. CHP contracts with these providers to maintain and build the network. While doing this, we also have an overarching goal to improve the quality and patient experience for the members served by the provider network.
