What We Do

Community Health Partners (CHP) is a Collier County based, Florida nonprofit healthcare organization. Community Health Partners is the liaison between major employer groups, physicians, health care networks and insurance companies. We are committed to bridging the gap between health care agencies, providers, patients, and their families. It is our goal to provide a seamless exchange of services and collaboration to ensure they have cutting edge health care options and a continuum of networks, providers, mental/behavioral health, medical services, and advocacy.

  • Direct contracting with self-insured employers

  • Contracting with insurance companies, networks, and health plans

  • Credentialing of providers using the Nationally recognized criteria

  • Provide Emotional Wellness services to the community and major employer groups locally

  • URAC accredited for Case management/Utilization management/Disease Management

  • Participation in utilization management and quality assurance programs

  • Update payors monthly of new providers and demographic changes

Rights of Persons Served

You are in partnership with Community Health Partners when receiving services.

Your rights are as follows:

  • To be treated with dignity, courtesy, and respect.

  • To ask for, consent for and receive quality treatment and services.

  • To receive services regardless of your place of residence, sex, race, age, sexual orientation, national origin, ancestry, immigration status, gender identity, cognitive status, veteran status, disability, or religion.

  • To be provided with communications and services in your language of preference. CHP staff may utilize in-house staff or a language line or HIPAA compliant translation services to ensure security and accuracy of communication.

  •  To utilize or be provided with auxiliary aids for those patients who identify as hearing or visually impaired via Telecommunication Relay Service (TRS via 711), large print documentation, Braille materials, captioning services, screen reader software, voice to text services, and any services that the patient may request for their accessibility. 

  • To request utilization of ADA accessible office space and facilities. 

  • To request all documentation and materials be presented in your language of preference.

  •  To participate in the development, understanding and maintenance of your treatment.

  • To have reasonable access to records.

Your responsibilities are:

  • To supply, whenever possible, accurate information about illnesses, hospitalizations, medications, and other pertinent matters related to your physical and emotional health.

  • To respect the rights of all others and be considerate of them, including their privacy and confidentiality.

  • To communicate your needs, accommodation or preferences to your clinician/case manager as assigned.

  • To report physical or mental abuse.

  •  To fully participate in your treatment.

Community Health Partners rights are:

  • To report all suspected, disclosed, witnessed, and/or confirmed abuse of a child or vulnerable adult to the Department of Children and Families and other necessary agencies. 

  • To give notice of your involuntary admission to your emergency contact person.
